Standard casters lack the chemical resistance, noise dampening, and locking systems that clinical environments require.

Clean room casters should meet the ISO classification of the room they will operate in. Key requirements include non-outgassing materials that will not release particles, ESD-safe construction for electronics-sensitive environments, stainless steel frames for corrosion resistance, and sealed bearings that prevent lubricant migration. Soft, non-marking thermoplastic rubber and polyurethane wheels in the Shore A 75-85 hardness range are the standard for hospital resilient flooring including sheet vinyl, VCT, rubber, and linoleum. They protect the floor surface, roll quietly, and absorb shock during patient transport. Grey-colored wheels are preferred because they show no marks on light-colored hospital floors. Avoid hard nylon or phenolic wheels, which can crack and dent resilient flooring under load.

Total lock stops both wheel rotation and swivel action on a single caster using a foot-operated brake. Central lock is a system-level feature on hospital beds that locks or unlocks all casters simultaneously from a single pedal, typically at the foot of the bed. Central locking is critical for patient safety during transfers and procedures because staff can secure the entire bed instantly without reaching each caster individually.
